What is hospice care?
Four levels of hospice care, one team.
Every patient's plan of care is led by our Medical Director in consultation with your own physician, and adjusts as needs change.
During a crisis
Continuous Care
A higher level of care provided at the patient's residence to address a brief period of crisis, as defined under the Medicare hospice program.

Respite Care
Occasional short-term care at a contracted skilled nursing facility to temporarily relieve family members or caregivers from caregiving duties.

What is hospice care?+
Hospice is a special kind of care for patients with a limited life expectancy, and their families. Hospice care is about quality of life and is holistic and comprehensive in its approach. The goal is maximizing comfort rather than recovery or cure, so patients may live as fully and comfortably as possible.
Who is eligible for hospice care?+
A patient who has been clinically diagnosed with a life-limiting illness and has decided not to pursue curative treatment can be eligible. To elect hospice care under Medicare, the patient must:
- Be entitled to Medicare Part A
- Be certified by their primary physician and the hospice medical director as terminally ill, with six months or less to live should the disease run its normal course
- Sign a statement choosing hospice care instead of other Medicare-covered treatments related to the terminal diagnosis and related conditions
Who pays for hospice?+
Medicare Part A and Medi-Cal/Medicaid — Hospice care is fully covered provided the patient meets the eligibility criteria and care is provided by a Medicare/Medi-Cal certified hospice agency like Del Rosa.
HMO / PPO / private insurance — Most managed care plans and private insurance have a hospice benefit. Del Rosa Hospice will assist you in determining your benefit coverage and make sure you understand any potential out-of-pocket costs.

Where is hospice care provided?+
Wherever the patient calls home. This can be a private residence, skilled nursing facility, residential care facility, assisted living facility or retirement community, anywhere in the Coachella Valley.
What happens after hours or on weekends?+
On-call staff are available 24 hours a day, 7 days a week — after office hours, on weekends and on holidays. Our office is open Monday–Friday, 9:00 AM–5:00 PM, but a nurse is always one call away at (760) 933-3000.
Does support continue after a loved one passes?+
Yes. Our bereavement counselor provides grief support, intervention and education during hospice care and for 13 months after the patient's passing.

Each member is part of one coordinated plan of care — for the patient and for the whole family.
Doctors
Our Medical Director, in consultation with your primary physician, oversees and leads the team in developing a plan of care.
Nurses
Trained registered and licensed nurses provide direct patient care in your home or other care setting.
Hospice Aides
Hands-on support with activities of daily living: bathing, dressing, eating, personal care, toileting or incontinence care.
Spiritual Counselors
Spiritual care and guidance for the entire family, in keeping with your own beliefs and traditions.
Bereavement Counselor
Grief support, intervention and education during hospice care, and for 13 months after the patient's passing.
Social Workers
Counseling, support, and referrals to other support systems — including help navigating insurance and paperwork.
Volunteers
Trained hospice volunteers offer companionship, respite for caregivers, and help arranging transportation needs.
Other Professionals
Dietitian, speech, physical and occupational therapists are available when a need is determined by the team.
