Doctors

Our Medical Director, in consultation with your primary physician, will oversee and lead the team in developing a plan of care.

Nurses

Trained registered and licensed nurses provide direct patient care in your home or other care setting.

Hospice Aides

Hospice Aides can provide hands on support and help with activities of daily living according to the patient’s plan of care. Care provided can include bathing, dressing, eating, personal care, toileting or incontinence care.

Spiritual Counselors

Spiritual counselors can provide spiritual care and guidance for the entire family.

Bereavement Counselor

Provides grief support, intervention and education during hospice care, and for 13 months after the patient’s passing.

Social Workers

Social workers provide counseling, support and can provide referrals to other support systems

Volunteers

Trained hospice volunteers offer a variety of services, from providing companionship, respite for caregivers, and arranging transportation needs.

Other Professionals

Dietitian, Speech, Physical and Occupational therapists and other therapists are available to render care if need has been determined by the team.
